Document package

  • Application form. We will help you complete it.
  • Photograph (35×45 mm). Must be taken against a white background.
  • Passport scan/photo. All pages (pp. 1–31) laid out flat.
  • Driver’s license scan/photo. Category C or CE.
  • Invitation from a Polish employer, which we will arrange for you.
  • Medical insurance. Minimum coverage of €30,000, valid throughout the Schengen Area. The insurer must be recognized by the Consulate of the Republic of Poland. The policy must cover the entire validity period of the visa.
