Checklist

Visa application forms: child’s form signed by a parent and the parent’s form.
Child’s photo: color, 3.5×4.5 cm, white background, taken within six months.
Child’s birth certificate: original plus sworn Polish translation.

Kindergarten acceptance: original invitation or certificate on official letterhead with stamp and signature.
Parent’s documents: passport, employer invitation, or student record confirming legal stay and travel purpose.
Financial proof: bank statement, income certificate, or sponsorship letter confirming sufficient funds.
Accommodation proof: rental agreement or hostel/dorm booking confirmation.
Ticket reservation and travel plan if available.
Medical insurance: €30,000 coverage, valid Schengen-wide, covering the entire visa duration.
